--- Comment #11 from cwo <[email protected]> --- They're obviously not pinned tasks - pinned tasks don't have "Remove Icon", and there's both a Firefox icon and a Firefox task visible - a pinned, but closed task will be replaced by the opened window, so you wouldn't have the full entry and the icon. I also notice that you seem to have changed the theme. Could I ask you to try it in a new user account, and there just replace the task manager with an Icons-and-text task manager and nothing else and just try launching one of the default pinned apps?
