The biggest problem is you are using the OpenLDAP shipped by RedHat, which is very buggy and broken.

Oh? I had no idea this was the case. It seems that the version is 2.2.13. The
latest stable according to openldap.org is 2.2.38. I guess the differences
between different minor versions aren't so minor after all?

I suggest using the packages by Buchan Milne at:

or the pre-built binaries available from Symas Corp @ <http://www.symas.com>
