Answers have been given to 1 and 3. for 2 I would say experiment with the Focus Groups, find what you like. About 4, there is no rule but I use the birthdate name when available, and later names (adopted names, anglesized(?) names, nicknames, spellings are all AKAs. Rich in LA CA
