The [#xxxx] notation started during the past summer with a script that was developed in the dev team, and I've been mildly grumbling about it the whole time. (Note that this isn't much more than mild grumbling on my part.) Our lifecycle doc says to use "fixes" and that this automatically closes the ticket (which is no longer true).
