Is there any reason not to do a 'yum clean all' ?  If not, why isn't it the
first thing yum always does?

This sounds a lot like this bug:

I used the patch at the top of the bug report and my servers are working
well now. I did remove /var/cache/yum/* also, just to make sure everything
was clean.
