  That's exactly my point.  There are so many things I would
like to see done to improve MMORPGs.  IMO they can get just as
boring as a MUD, you just have something to look at it while you
do it.

  I based my MUD around video games mostly (Grand Theft Auto,
Mega Man, etc).  So I took out the whole elf, dwarf, etc and put
in real races - caucasian, asian, etc.  The first big
enhancement  I made was adding firearms.  Each firearm has a
specific type of ammunition it can use (shotguns can only use
shells, pistols can use 9mm or .45, etc).  You have to reload
each clip using a box of bullets, or energy source for lasers,
etc.  Different weapons have different recoil times before you
can fire again.  Of course you can fire from room to room.  
  Another big "thing" I coded was an even better automated
auction system.  Instead of just auctioning player items, the
auction system (appropriately named eBay) throws up an item from
the house.  It pulls a random item from the ebay.are file. 
These items have rare and uncommon flags (talk about Magic the
Gathering :P) which also affects how often an item is auctioned.
  The only other noteable thing I've done is basically what you
said about your item system.  Mobs repop with items as normal,
but based on your character's luck attribute (affected only by
objects, default at 0) you can find a unique item.  I made a
table of "unique" prefixes and suffixes, and the stats and
ranges of applies, and level associated with each.  When the
unique item is created, it randomly selects a prefix and suffix
(assuming your level is high enough for each one) and then rolls
for the applies.  For example, at level 1 you might only be able
to get a Brilliant or Shining prefix, which are 1-5 hp each, but
at level 10 you can now get Raging which is +1-2 damroll.  I
copied this idea from Diablo 2 :D
  Those are the few big ones... although I haven't really coded
anything for my MUD in years since the population is... oh yeah,
0. ;)
