Siva Saravanan The use of digital computer devices in analysing and reporting our body functions is becoming popular Professor Eric Topol of Scripps Health, San Diego, CA has written the book "The Creative Destruction of Medicines: How the Digital Revolution will Create Better Healthcare". I chanced to read not the book (yet), but my colleague Dr Sreedevi alerted me to his interview by Ms. Lucy Mckeon; it is exciting with possibilities of the use of digitalisation in medical diagnosis and treatment. The use of digital computer devices in analysing and reporting our body functions is already becoming popular. Indeed, there is an entire two-hour session devoted to the use of "smart phones" in ophthalmology, in a forthcoming vision research meeting in Florida. (Do not forget that the cell phone is a portable digital computer in itself). Data storage is now done in devices smaller than the "bindi" that our ladies wear on their foreheads; perhaps, her pulse rate, heart beat and such can be monitored using the bindi. Dr. Ramesh Raskar of MIT has already devised an "app" in a smart phone ( i-Phone in this case), which allows the user to check his/her eye "power". This replaces the traditional eye chart, and allows the user to go directly to an optician and obtain appropriate spectacles or contact lenses. Contact lenses themselves are suitable surfaces on which tiny digital devices can be embedded, with no hindrance to the vision of the user. A group in Spain has put in a pressure-sensing device on the contact lens, which measures fluctuation in the intraocular pressure, real time, thus offering diagnosis for glaucoma. The patient is not needed to come to meet the doctor in the morning and evening just to register the pressure. Another group in Seattle has embedded an entire integrated circuit system on the contact lens. This is powered by the battery of a cell phone in the pocket of the user, with which it communicates via Bluetooth). This device is thus able to access directions via the GPS facility that the phone downloads via the internet. Imagine how useful this device would be for a blind person. The pocket ultrasound device is another revolutionary one. General Electric, among others (even in China) is marketing this device, no bigger than a Blackberry phone. Dr Topal says "I haven't used a stethoscope in two years. I use this pocket ultrasound device so I can see everything in the heart rather than listen to the heart's sound." Dr Topal makes special mention of the use of the data obtained from the human genome project. It is now possible for a person to get the read-out of the entire of 3.2 billion units-long DNA sequence for $20,000, and soon it would be possible to do so for $1000 (Rs. 50,000). And this is but a one-time investment, which can come in handy (as my colleague Dr Santosh Honavar remarked, get it done the moment the baby is born - a genetic horoscope, as it were). And this entire personal genome sequence can be stored in a smart card/cell phone memory. Of what use will this personal/ individual specific human genome sequence data be in medicine? With data already available, we can predict whether a given medicine is safe or not, or suitable for you, based on your genotype. Drugs like Metformin for diabetes, Plavix for blood clots, or Tegretol for neurological disorders, are not acceptable for some people because of their genetic background. Thus if we already know the latter, we can prescribe more acceptable alternatives. Dr Topal further points out that there are a lot of people with debilitating, serious illnesses, the causes for which are yet unknown; neither the diagnosis nor the treatment. With the knowledge of the whole genome sequence, he believes that we can determine the root cause and the biologic bases of such "idiopathic" illnesses. When Ms. Mckeon asked Dr Topol what technological innovation in medicine he is must excited about, he said it is the embedded nanosensor. This would be a nanoparticle-based device that can be injected into your blood stream and made to localize in the wrist, finger or some such place from where it can communicate with your cell phone. " And that embedded nano-biosensor can be used to pick up, for example, the first cancer cell that shows up in the bloodstream, which would promote the earliest possible detection of cancer. It could also help with the autoimmune form of diabetes, called Type I which usually affects kids... (or) it can be used to pick up cells in an artery that's starting to crack a week or two before a heart attack (i.e. prevention). The disability sector of India has also started looking at civil aviation policies and rules once again, started discussing and demanding changes that should be incorporated. It is ironical that both Jeeja and Anjlee work for including disability in mainstream with one of them focusing on accessible environment. It is humiliating and painful for any disabled person to face this while travelling by air. But a greater irony is the fact that we are forgetting the large number of disabled population who face harassment in travelling by any mode of transport on a regular basis. And here I am not talking about physical accessibility of buses, metro, trains, trams or any other public transport. I am talking about the attitudinal barriers they face when they try to use public transport. It is important to remember here that thousands of disabled people in India do not even dream of boarding an aircraft in their life-time. One does not have to be an activist to know that poverty and disability go hand in hand. While most of us spending our time in thinking of making “skies” inclusive, let us give some time to make the ground below our feet more inclusive. Endless cases are heard about attitudinal barriers disabled people face while travelling in a bus. Only once we could make it to a newspaper headlines. February 28, 2012 A University of Minnesota researcher is developing an app that would tell the blind and visually-impaired not only when to cross the street, but which direction they're going and how many lanes they have to cross. The app, which will be free to download, will also tell users the name of a street if the user taps the phone and points it in any direction, said Chen-Fu Liao, a senior assistant engineer at the University. Users tap the phone again to request a walk signal so they don't have to struggle to find a button to push. The system will tell users when to cross and how much time they have. Currently, Accessible Pedestrian Signals help visually-impaired people know when to cross a street with a countdown. Liao works at the Minnesota Traffic Observatory on several transportation studies using Intelligent Transportation Systems and has developed web-based simulations for transportation courses. Research for the app began two years ago with a pricetag of about $100,000 each year, Liao said. To get started, he and a colleague interviewed 10 visually-impaired people about what they'd like in the product and what format would work best. "I'm really excited about the whole project," said Ken Rodgers, president of the American Council of the Blind of Minnesota. Rodgers said while there are lots of benefits to the app, there are also drawbacks. While it gives visually impaired people more information than does the APS system, not everybody has a smartphone. He said the more types of platforms that the app works on, the better. The app will eventually tie right into traffic technology and have the potential to alert drivers that a visually-impaired person is in the vicinity, Rodgers said. The app is a complement to the APS system but is a completely separate tool. Rodgers said he's afraid that if the app gets too popular, cities might stop installing APS. Signals and traffic crossings owned by the city, state or county are obligated by federal Americans with Disabilities Act regulations to make the visual elements accessible to people that can't see. "Right now, they do that by installing Accessible Pedestrian Signals," Rodgers said. The app will not relieve cities of this obligation to provide alternative ways of accessing the crosswalks, he said. The prototype of the product will be tested in Golden Valley, Minn., beginning in late March or early April. Linda Spaulding, a certified orientation and mobility specialist, chose the testing sites. The crosswalks needed to have stoplights, and one has APS while the other does not. Both needed to be close to the Twin Cities, she said. Researchers wanted the testing sites to be close to the Twin Cities since the people involved in the testing would be visually impaired and would have difficulty traveling far. Spaulding also worked with the Minnesota Department of Transportation to determine which intersections would be safest to use. There isn't yet a launch date for the app. Research will continue to improve on its current prototype by looking for ways to help prevent users from veering while crossing the road, Liao said. A bigger version would include door-to-door guidance that would help users get to bus stops, know when buses are coming and when they're approaching. "I think the app is totally cool," said Rodgers. "It will really help us navigate the streets." Hi sir, yes it is true that in order to get your book scanned from that scanner you will have to cut the book . and regarding your 2nd question surely it has better resolution . Hello friends. I have been using Kurzweill1000(v12 now) for scanning my books over the years. Now I have access to finereader11 and openbook9 as well. I would like to know as to which program performs better in terms of scanning quality as well on accessibility. K1000 is using finereader9 along with scansoft 16, but between finereader 8 and 9 used in different versions of K1000, I could see a real difference in quality. Hi Krishan, Download NVDA from the link pasted below. http://dl.dropbox.com/u/8377769/nvda_2011.3_installer.exe and install it. After installation press insert + N to get NVDA menu, press down arrow once to select preferences submenu then right arrow once then down arrow twice to select voice setting dialogue box, press h three times and you will hear hindi, press enter. Now, you can read hindi text. To write Hindi, after making above setting , open MS Word press left alt with left shift key and NVDA will announce layout devnagari enscript. Now you can enjoy typing hindi. You can toggle between hindi and english typing by pressing left alt +left shift in between as per requirement. Feel free to contact for further guidance. So one does not feel surprised when within few days of Jeeja Ghosh incident, another disabled activist Anjlee Agarwal faces humiliation while flying from Delhi to Raipur. We are aware that these experiences are part of lives of disabled people. We are proud that Jeeja and Anjlee have the guts to put up the fight. The disability sector of India has also started looking at civil aviation policies and rules once again, started discussing and demanding changes that should be incorporated. It is ironical that both Jeeja and Anjlee work for including disability in mainstream with one of them focusing on accessible environment. It is humiliating and painful for any disabled person to face this while travelling by air. But a greater irony is the fact that we are forgetting the large number of disabled population who face harassment in travelling by any mode of transport on a regular basis. And here I am not talking about physical accessibility of buses, metro, trains, trams or any other public transport. I am talking about the attitudinal barriers they face when they try to use public transport. It is important to remember here that thousands of disabled people in India do not even dream of boarding an aircraft in their life-time. One does not have to be an activist to know that poverty and disability go hand in hand. While most of us spending our time in thinking of making “skies” inclusive, let us give some time to make the ground below our feet more inclusive. Endless cases are heard about attitudinal barriers disabled people face while travelling in a bus. Only once we could make it to a newspaper headlines. Bidyut Dey, a 50 yr old man with amputed leg was thrown out of a Government bus as he said he has the right to travel without tickets. Dey himself a West Bengal Government employee is an organiser of sports of disabled people. He travels all over India with a cricket team comprising of disabled youngsters. He refused to let this incident go unreported and lodged a FIR and followed up the case regularly. That he was ridiculed by Police for making such trivia a case, and the Magistrate who was listening to his case was shocked to find that a man can refuse to buy tickets while travelling and say that this is his right, is another story. Neither the Police officer nor the magistrate was even aware of a law called Persons with Disabilities Act after 12 years of enactment. These are the facts we have learnt to accept. Like

The limitation of REPLACE() function when copying it is that the strings to pirge should be of the same length and start at the same position. The use of these functions depends on the situation you are in. For single values, they always produce the desired result.
