Something caused sheer, but only sometimes. Wearing jeans 1 day is what
caused the sore. Doc said it looked like sheer, not pressure.

Same wound on my ishial, caused by shearing,i.e., my mattress is made up with
air pockets and the bed has multiple movement settings. If I bring my head up
without raising my feet first it causes a "shear" problem between my skin and
skeleton. Picture your skin as a sack that. Houses your skeleton.

Never had any breakdown until the VA gave me the bed. Also, I have a ROHO
Contro Select cushion that has my ishial suspended over a hole so it provides

Re: [QUAD-L] Sore on rear end.

2015-05-18 Thread Robert Vogel
Hi David,

Larry Willis’s advice is right on the money—every point should be followed.

1. Ditch the egg-crate for a high profile Roho. Make sure it's high profile.
2. Stay off your butt as much as you can.
3. Consume as much protein as you can. Very important. Consider supplements.
4. See a nutritionist, have analysis done to see if you are short on any
nutrient.
5. Keep seeing wound care folks so they can monitor.

Adding my two cents: When I was first injured, back in 85’, I asked for a ROHO,
instead I was sent home
on a memory foam cushion. Despite constant weight shifts, within 6 months I
got a small pressure sore
(size of a pencil eraser) on my right ischium. I spent three months staying
off of
it and trying to heal it on my own-- with no improvement. Then I got my doc to
write a prescription for
a high profile ROHO, and referral to a wound care clinic. The combination of
the cushion and wound care
specialist got the sore to heal within 1-month.

Now, 30 years post injury, my butt is skin and bones, plus I have a pelvic
obliquity (my right ischium sits 1 1/2” higher
than my left)-- so bad that the left ischium gets red after sitting up in bed
for a couple minutes. I protect my skin by
using a ROHO Quadtro Select high profile cushion. Nightly mirror skin checks
every show that the ROHO protects
my skin, with no red areas.

The/ A wound vac, is merely an instrument, hopefully in the hands of an
artist is a great tool. Besides the wound vac are various accessories that
must be used. The filters, the patches and powders. It all begins with the
wound being properly debreded.

After the wound has been reduced and healing, a flap surgery procedure can
be considered.

I occasionally get a sore throat in the morning. I think this is probably
because of oxybutynin, which I take an extended release pill now. I've had
a lot more problem with a sore throat lately, but I just have some water
close by and sip a little bit. This usually takes care of it.

two weeks ago I had a skin graft and I've had to keep my heel offset, since
May 4, when the surgeon left the external fixater on my heel too tight and
did not check for seven days after foot drop surgery. I lost some of my

heel problems.

I will try to get my friend to take pictures so you can see what was made.
They work wonderfully.

The company my orthotist works for is hangar's, and I don't know if they
are nationwide. No one at KU could figure out what to do. The OT's for the
orthotist that KU used which was the same company. This orthotist is just

RE: [QUAD-L] sore

Additionally you should keep your heels and ankles elevated with either heel
lifts or pillows under your ankles... I'm surprised it's on your ankle and
not your heel.

While its healing try to either keep all pressure off it or rotate pressure
elsewhere by turning.

It's easier to prevent skin breakdown than it is for it to heal, you should
try to make it a routine to check for early signs of pressure sores or if
you're like me a c2 quad have a caregiver or family member check daily. My
cargivers do it while I do my range of motion exercises.

sounds like a bone infection and have many of them over the years. A simple
blood test can tell if your blood count is off and also possibly taking of the
area itself. I spent over two years to convince my doctor about a bone
infection but did not listen to me! I located some doctors a couple hours away
and they quickly determined that I had a bad bone infection. No surgery on the
outside area is going to keep it closed, if there is infection in the bone. Any
other questions feel free to ask and yes it took surgery to resolve my issues

It's a year now since I got my butt sore, well, just next to my tailbone. I
can't recall the exact numbers, something like 2.5 cn x .6 cn.
It was not too deep, but then tunneled towards 1 o'clock. It slowly healed
about half way in 8 month. Then within a week it just went back to the
beginning.
We pack it with Collagen AG. We tried a few different things, but nothing is
helping.
I assumed the sore was from sleeping on that side yet changing sides has not
helped.
Then we thought it had to be from the back of my chair. The padding was flat
and the bottom of the chair's back was real hard. I got a new pad and lowered
my cushion so the sore area now does not touch the back. There is now a 2 inch
gap between the top of the cushion and the bottom of the chair's back.
Yet this sore just will not heal up. The doc is suggesting starting to think of
surgery.
He thinks it might have started as a Pilonidalcyst, but there was never an
infection or anything.

Greg, make sure you are getting a truck load of protein every day along with
other vitamins etc. Get at least 60 g. If possible. The nutritionist made a
dosage chart for me and it made a big difference getting my butt wounds healed.
The wound vac was a big help too. Good luck, my friend.

I'm in a Permobil and I hate the back of the chair. I had used a LaBac for

Greg,

Merrill is probably right. Yeast infection. I've had that once before too.

I don't believe I took antibiotics, but I did use a prescription anti-fungal
cream.

You should definitely see your doctor as soon as you can, but until then try
using an over-the-counter antifungal cream or ointment.

It won't clear things up, but might keep it in check until your doctor can
prescribe something stronger.

Also, there are natural antifungals, if you want to start down that route.
I was drinking grapefruit seed extract [GSE]

for a while, but it's so bitter that it's literally difficult to swallow.

Get well soon,

Don.

High bacteria will do that, often caused by a combination of humidity, dark
areas of the body and human body fluids. By all means, let your doctor

!st, you must get off that spot. Rolling or propping somehow untill you can get
a low air loss mattress. While your off of it you may want to start with some
triple anti-biotic ointment. If the skin is not open, you can go to
hydrocortizone creme. Do not sit on Underpads!
I'd mention tripsin to your doctor if the wound is clean.
Do not use hydrogen peroxide on the wound. If you can stand getting on your
side, place a damp gauze patch on it and let it dry. You can speed drying by
using a small fan. (simple wet to dry treatment)
If you decide to try this, be sure to boil the water and let it cool.
You might be able to reduce your breathing problem with proventil.
I do wish you luck. Weekends are hard for me to get help too.
Fortunately, everyone has a cam on their phone so, you can get a picture and
see what your dealing with.

see if it's possible for you to get a low air loss mattress.  I had a friend of

If you have problems like I do, you need to make sure you're taking something
to loosen the secretions in your chest.  Talk to your doctor about the generic

chair to keep my airway clear. This morning during our daily butt check a
weeping sore was found on my tail bone, this is unusual for me since I almost
never have skin issues. It is Sat morning no VA sci unit open and no sci nurse
to call. Does anyone have any suggestions as to how to treat it until Mon.

I know, being a quad for almost 12 years I should know, but really, I just
don't have skin issues and I don't want to be on my stomach for 6 weeks.

My original cushion (13 years old now) is still my favorite one, but we have
to fill it with air every day and allow it to go back to room level.

I received a new high profile Roho a while ago and simply tried to put air in
it and then let it get to where it was room level. It just did not have the
same amount of air as the older model. Even though it's the exact same model,
I sit significantly lower than I do in the older one.

I have had a hard time getting the newer cushion properly adjusted. I
thought I had it and then I decided to let out a little more air.
Unfortunately my legs started to splay too much and this tells me that the
condition is improperly inflated.

Does anyone know where the website is that demonstrates how to properly
inflated these things?

I think you'll be ok now that your cushion is properly inflated. I had a
sore that I got in rehab over the summer heal while sitting on my roho after
they got me on an air mattress at night. Its the other sore that got infected
and lead to another surgery in almost the same place as the sore I got fixed
last year.

When I met with the wheelchair dealer for a new chair, he said that he has
seen people heal on a roho cushion. As long as it has the proper amount of air
in it and pressure relief is being done.

Speaking of pressure relief, since I got this surgery fusing my back even
more, I am not longer able to get any sort of shoulder deppresion that leaves
me unable to get my butt off of the surface. I lean now. I haven't gotten any
new breakdown since the episode over the summer but am curious how you all do
your pressure relief.

2008-12-12 Thread John S.
Your exactly right on the 1 inch from bottom at lowest point. with the quatro
seat, you really should be sure it is locked. I have an aide that sits in it
each morning because she knows what it should feel like. Anytime you get up and
feel like you are low,, stop and check the roho. The worst thing about them is
that I'm only sitting on three shots of air from their pump. A very small air
loss can turn a roho into a disaster.
As for your bum, get off it now and stay off untill the bruising either fades
or your open a sore. Ask your doctor about using oxygen and maybe some
hydrocortozone cream (2.5 strength at least) on the area three times a day.
It is critical to shorten healing time by fast initial action. If you continue
to put preasure on the area it is far more likely to lose more tissue to
necrosys, god forbid that you lose any.
Invite over some folks you know, deacons at church, whatever, the boredom
sucks, but get off your butt!
Nows your chance to learn mouthstick artwork!

Best of luck,

Ditto on the being intrigued.

I have a fine line between an over-inflated and under-inflated Roho (hi
profile/quadro select); due to my skinny bum. The only thing I can think of on
how a smaller cover would help, is, it would help hold the cells together from
separating, which mine does and allows my bum to get closer to the bottom of
the cushion.

So what size is your cushion Merrill, and the smaller cover?

cushion cover size makes a BIG difference. a
smaller (tighter) cover allows you to have less
air in the cushion. proper inflation for a roho
is to blow it up and let it sit open valve to
room pressure. then cork it up and put into a
cover that allows you to sink in but not bottom
out. I've never had a pressure sore from sitting on a roho.

I think you'll be ok now that your cushion is
properly inflated. I had a sore that I got in
rehab over the summer heal while sitting on my
roho after they got me on an air mattress at
night. Its the other sore that got infected
and lead to another surgery in almost the same
place as the sore I got fixed last year.

When I met with the wheelchair dealer for a new
chair, he said that he has seen people heal on
a roho cushion. As long as it has the proper
amount of air in it and pressure relief is being done.

Speaking of pressure relief, since I got this
surgery fusing my back even more, I am not
longer able to get any sort of shoulder
deppresion that leaves me unable to get my butt
off of the surface. I lean now. I haven't
gotten any new breakdown since the episode over
the summer but am curious how you all do your pressure relief.

So what size is your cushion Merrill, and the smaller cover?

cushion cover size makes a BIG difference. a smaller (tighter) cover
allows you to have less air in the cushion. proper inflation for a roho is to
blow it up and let it sit open valve to room pressure. then cork it up and
put
into a cover that allows you to sink in but not bottom out. I've never had a
pressure sore from sitting on a roho.

I think you'll be ok now that your cushion is properly inflated. I had a sore
that I got in rehab over the summer heal while sitting on my roho after they
got me on an air mattress at night. Its the other sore that got infected and
lead to another surgery in almost the same place as the sore I got fixed last
year.

When I met with the wheelchair dealer for a new chair, he said that he has seen
people heal on a roho cushion. As long as it has the proper amount of air in
it and pressure relief is being done.

Speaking of pressure relief, since I got this surgery fusing my back even more,
I am not longer able to get any sort of shoulder deppresion that leaves me
unable to get my butt off of the surface. I lean now. I haven't gotten any
new breakdown since the episode over the summer but am curious how you all do
your pressure relief.

to keep it clean, keep it dry, and keep off of it so it doesn't rub. A major

I have an area 1 inch by 1/2 inch, not deep, good granulation, VNA checked and

The problem is that it is under the scrotum where it meets the torso, so
it is always warm and damp.

The VNA has me on protein, vitamins, etc. We have used Progran, a thin

Larry,
One thing nobody mentioned is to sleep at night with your legs in a frog
position. That is with your knees bent and legs apart. It will keep air
circulating and your skin dry in your sensitive area. Every little bit of
goodness

helps. Good luck with that tough decision. Do you live in the DC area by

I don't know if you can heal the wound and still get up 12 hours a day. I know
it is hard to choose between a job and healing a wound. If there is a way you
can work from home and cut your hours back in the chair. I nursed a wound for
the last three years that I worked and it was a major reason that I gave up
working and became my useless lazy self. If I could change anything I'd have
retired to arizona or nm. I would have begged for my job if it had meant as
much to me, but I was tired. I don't wish your choice on anyone. I don't think
anyone makes a low air loss seat cushion yet, but that would be a major leap in
seating no insurance would pay for.

good luck with whatever you decide.

I don't think it will ever feel if you continue to get up. My sores will not
and I always go to the hospital wounds Center where they can correctly monitor.
They also give me the necessary
supplies and treatment. The used a waferlike material called fibercol that was
very expensive. You would cut smaller than the wound and put inside and cover
with a gauze. They would debried every couple weeks. I remember it taking six
months to heal and I was in bed about two years ago. It was caused by a laptop
power cord getting overheated as I switch from side to side and move the
laptop, being under my lower bottom where your leg and bought meet. I would
just get up long enough to wash my hair. I got really sick of it and would've
gone crazy with out my laptop. Ha ha

Re: [QUAD-L] Sore

2007-02-04 Thread Dan Tessiatore
Try to keep the area from being overly moist as your wound will not heal if to
wet. Naturally, the more you stay off it the quicker it will heal. I had a sore
in a fold of my skin on my behind and finally decided to stay in bed off of it.
It does help. Possibly look into a cushion that would provide relief for that
area. Have you or your caregiver consulted with a wound care specialist? What
are you sitting on? my wound care specialists recommended using a new products
manufactured by Smith and nephew. Using a product called Solosite on the open
wound and covered with a gauze bandage. On the healed areas and at risk areas I
use a product called Extra Protection Cream Hang in there it will heal. Dan T.

Question for the guys out there. (different anatomy than the female)

I have an area 1 inch by 1/2 inch, not deep, good granulation, VNA checked
and can find no tract, but it just won't heal.

The problem is that it is under the scrotum where it meets the torso, so
it is always warm and damp.

The VNA has me on protein, vitamins, etc. We have used Progran, a thin
covering that you put over the area, wet with saline and it is absorbed
into

the area. No change.

I have now gone to a treatment that we used before, putting a heatlamp for
3

minutes along with neosporin and bagbalm in the AM, up in the chair 4-5
hours, then put a fan on it for 7 minutes, then neo. and bagbalm. No
change.

Any ideas?

I have a large sore on my ass that I have been trying to heal for over 3
years. I has no depth, just open flesh about 3x4 in. I've tried every
miracle cure known to man and a few unknown. It won't heal. The problem - I
have a full-time job and have to sit 10-12 hrs a day. It depresses the hell
out of me and I don't know what to do about it.

GA...

Is there any way you can stay out of the chair long enough for the sore to
heal completely? It would seem that spending even a couple of hours let
alone 4-5 hours in the chair daily would undo any healing that is
accomplished when you are not in your chair. I have had periodic skin
trouble for nearly nineteen years of injury and I have never been able to
heal a problem without staying out of the chair. I also keep a small fan
blowing on my groin area any time I am in the bed, sore or otherwise, to
prevent moisture from building up and creating problems.

Correct me if I'm wrong but bag balm should not be used on open surfaces.

I had a big lump in that area last year that broke open. The doc cleaned it
all out and packed it with some seaweed stuff it never closed. He had
to go back in and scrape out all the [I forget the medical name] "bad" skin...
'dirty wound' I think he called it, stuff that would just never heal. After
he scraped it out, he closed it and after a tiny bit needed lancing a month
later, it finally closed and remains so now. That was an ugly 4 month trip I
don't care to repeat.
Best of luck to you.

I tried miracle mist and it didn't do anything for me.  It is probably
because the sore is that bad.

RE: [QUAD-L] sore

2005-12-16 Thread Lana Baugh
Gabe had a pressure sore on his butt that he couldn't kick. We tried all the
heavy duty "derm" dressings. We found out that sweat and the general
bunching up of the buns when sitting and dressings falling off early kept
the sore alive and well. So we tried Tegederm. Yes, the thin stuff. It
worked. I think it worked because it followed the butt crease in sitting and
laying and the skin could breath easy so it could stay on for the optimal 7
day period.

Most of the stuff I've read and info at conferences says that wet to dry
dressings actually keeps the site from healing. Every time you change a
dressing it kills and rips away the new tissue. So whatever dressing you
use- foam, the many different duoderms, silver laced dressing, miracle mist
etc- they need to stay on long enough for healthy tissue to form and get
established.

Have you been to a wound care clinic, had wound care nurses help, or had a
wound care docs help?

The PVA has a great book on SCI. It has a lot of good info on pressure sores
& Treatment. A good source is also the American Spina Bifida Assoc. A lot of
individuals with SB fight with sores their whole life. Their parents fight
for better care when they are young. So they get pretty good $ to
investigate pressure sores. I went to a conference about 15 years ago and
went to a session on presentation sores- wet to dry was out and the derms,
foams and sprays were in. From the care Gabe has been receiving I think it
is still the case.

Hope this helps,
Lana

I'm so glad they're healed .  Congratulations.  I ordered some
miracle mess that I have not used on any sore because I have not had any.
I did use it on a small scrapes.

I'm glad it's good stuff.  You're a testimony to it.  You should
write the company about your success.Dana

Hello William,
Ive had two sores on my butt for 2 years, been going to a wound care
facilitie. They tried everything, even a wound vac. Which made them
worse! Someone on the list told me about miracle mist, I figured what the
hell and ordered a bottle. With in two weeks my Dr was freaking out at
how well they look, I finally told him what I was using. Of course he
laughted it off, after the next few weeks he wonted me to bring the bottle
so he could see what was the ingredients. Within 4 months the wounds are
just about gone.
