Set the Deleted Item Retention time for messages to 0 and IS maintenance to always and let that puppy run for awhile. Also, check the 1221 events in the app log and see how much space has been freed up. The 1221's will give you the min amount you can reclaim after defragging with eseutil again.

I would suspect the system is replaying the queue.dat file found in the imcdata folder. I would hazard a guess that all the messages being replayed if you will are all SMTP. Stop the Internet mail service then rename/move/delete the queue.dat file and then restart the service. This should

Run the Task Manager on your Exchange Server and see what's eating up all the CPU Pocesses. We have this problem occasionally with mssdmn.exe running 100% utilization. We just kill this process and things go back to normal. As of yet, no one at MS has been able to explain why this occurs.
