I solved this problem.  For those who are interested:   1) Apparently, the DatabaseServerLoginModule does not like underscores in the application-policy name,   and/or   2) The value of the role group column must be 'Roles'.   Hope somebody besides me can learn something from this.

ditto. We had java talking to active directory via ldap no problem. The only issue we ran into was that active directory had a nasty habit of self destructing and serving up the blue death. It might be worthwhile to consider a couple of other directory or database alternatives just in case.
