Tom, I do not have the knowledge to help solve your mysteries, but if I have any problem with any record, I find it interesting to see the date when the individual was added, marriage added and when modified. Might help to see if the record was imported, or if all problem records were modified

Donna, Sorry, I'm still confused. Where are you expecting to see the access date? The access date is always part of the source detail and not part of the master source. The publish date is part of the master source. I guess I'm also not understanding what the detail field for the detail means.
