Instead of holding down the option key, hold the letter U down.
another thing is, unplug your drive, boot it up, and instantly as soon as
you hear the chime, push the drive into the port. I've found that some
times, the drives don't get power right upon the immediate boot u
ctrl+Alt+O should launch Orca, so in apple terms, command+option+O.

I should add that I did test on the IPhone and do not have this issue. I

Remember the iPad is devided in to sections so you have to scroll the
screen in order to see what voice over does not.
