I set it up according to the Readme. Everything is in the XF86
configuration file that supposed to be. When I startx with KDE open
xterm type imwheel -k it says its running. I Open some windows in KFM
and try to scroll them and I get a menu like the right click menu. If I

Ok, this may not be the answer you're looking for but, I copied it from
/usr/X11R6/bin/imwheel to the autostart folder on my KDE desktop and it
runs at bootup.

imwheel -k
in your .xinitrc

It'll work if you use desktopcfg, i assume t'was a blank .xinitrc so you
created one?
gnome also has session management, you can add 'imwheel -k' in session
control of gnomes 'control-center'
