Give a try to dovecot.
Use qmail-ldap only for SMTP, dovecot for POP/IMAP, both authenticate
using the same ldap but they use their own configurations.

I startup courier-imap independently of Qmail. I use the startup scripts
with chkconfig on Redhat to start up authdaemon and courier-imap.
As far as I know, there is no other way...

I've got courier-imap working nicely with courier-imap. No problems
with folders or sent-mail. Auto-save works out fine to (it puts mail
in the drafts folder). Using courier-imap 4.1.1.
I remember that Mail.app was very picky if there was wrong

and LDAP_FILTER (if you use courier-imap) in conjunction that could
