Sounds suspicious for malware. Check the logs (var/log/qmail/smtp/cur and the similar one for the submission port) to see if email is coming from outside the system. If it is not from outside the system, then I would go looking for some sort of rogue running process (likely malware) which is

Thanks to Eric for pointing me in the right direction -- upon CLOSE inspection, the remote queue itself was owned (properly) by qmails, but the queue FOLDERS within were owned by qmailq -- no idea how that came to pass, but it has been repaired and my mysterious queue errors appear to be gone.

No, it simply sends output to the terminal. I suppose that a customized version for QTP which would tee off a log file automatically would be nice. That would be pretty simple to do with a script wrapper. Would anyone like to take a shot at writing a qtp-queue-repair script that would do this?
