For reasons unknown nearly every car stereo I've looked at the
specifications of that can play CDs of MP3s is limited to ID3V1 tags.
These are years out of date so it's really about time they got their

Yup, mine runs every night to ensure that my MP3 library properly
mirrors my FLAC library with all my tags, cover art, etc. properly
maintained.
And if I'm really in a hurry to get something onto an iPod I also just
kick off a conversion of a particular album manually.

My main library has over 1000 albums in it in FLAC format with rock
solid behaviour from the tags and covers, but it did take a bit of work
up front to ensure that the tags were consistent and correct, and a
decision to go with embedded
Do you have more than one tag type in the file?
For example MP3tag will quite happily write ID3v2 tags and leave ID3v1
and/or APE tags behind in an MP3 file unless you've told it
specifically to delete the old tags.

mp3tag will work on multiple files and directories at one time, so you
could temporarily move the affected folders into one parent folder and
let mp3tag scan that, highlight all files, remove images, save, and
then move the folders back to their proper locations.

MP3 and FLAC files.
Lots of people here use it and have expertise in it, so if you're new
to tagging it's a good one to be using as it'll make it easy to get
some support from this community.

I'm fairly sure DVD Audio Extractor can take an ISO file as source, as I
believe I've done that before with some of my concert DVDs.
There's a fully-functional 30-day download available but,
unfortunately, it's Windows only.
I don't know of a Linux solution
Poikosoft Easy CD-DA Extractor in correct and repair mode works really
well with copy-protected CDs.
I bought it back when I first got an iPod and was struggling to get a
number of copy-protected CDs on and have been using it ever since.

If you're on DOS then the output goes to stdout, so you can append a
>filename to the command to log all of the output to a filename of your
choosing for review afterwards.
Your command will be something like:

Scan for new and changed should be all that you need to do when you've
been editing tags. I always change the timestamps on all of the tracks
in the same album to ensure that it gets picked up, and then it'll
effectively be treated as a new album replacing the old one.

artwork from Easy CD-DA Extractor (or MP3TAG if I update the image) and
it works perfectly,

We have a winner!
Just got that modified version of flac2mp3 up and running in 5 minutes
flat on my Debian machine. Another 5 minutes to get the tag mapping
perfect for ARTISTSORT, ALBUMSORT, etc. and I'm really happy with the
results.

keep the shadow library up to date without doing unnecessary
conversions.
What turned me off originally was that every thread I came across
mentioned embedded cover art as a limitation. Has this now been
resolved?
I'd like something that uses the LAME encoder preferably so I can set
the quality options I'd prefer (i.e. as close to FLAC as I can get but
still portable if I need the MP3s for other uses, so Apple Lossless is
out). Windows, DOS or Linux is fine.
