Was setting up a new 4.2 system (installed from ISO) when I noticed that
my Snom phones were not auto provisioning - for some reason it appears
that the required line " option bootfile-name
"/phone/profile/docroot/{mac}.xml"; " is missing from the dhcpd.conf

You came across a real problem. Basically, with XMPP, the call will only work
if the caller has sufficient permissions to make the call whereas with
conventional call forwarding rules, the permissions of the callee get used.

You need to go from 3.10 to 4.0 first, you need to send the server profile
and reboot the server. I've seen that too and had to go through each rpm
finding that some had not been installed.

No, as I said, I had similar problems going from 3.10 to 4.0. Essentially the
sipxsupervisor RPM didn't get downloaded. Had I had more time I would have been
able to figure this out and understand it. However as I only have a limited
amount of time to get this done, I figured I'd try 3.10 to 4.2
You stopped and ensured everything was working in 4.0 before going to 4.2
and sent the server profile and rebooted the server?
