This is actual a Linux issue rather than a CloudStack issue.
When you clone a VM there are various steps you must take prior to the actual
clone (which is what a template is)
Suggest you google for your combination of Hypervisor and VM OS as there are
plenty of 'how to' guides online.

I spent a couple of days to make Cloudstack 4.2.1 working with XenServer 6.2,
with the option to use local storage for systemVM.
To do so I follow this steps :
1. Do not download vhd-util as said in CloudStack 4.2 install guide, but use
one provided by the XenServer 6.2 ISO
I used the systemvm template from 4.2.1 install guide.
I have no idea how to check if it's stale or not. However, it started
working after reinstalling ACS 4.2.1, the problem occured with CCP 4.2.1.
I'll reinstall the platform entirely next week, and if it happens again
file a bug for it with Cit
Check whether you are using the right version of system vm template and
the system vm isos are also not stale.
Generally I see this happening when the above combination is not right.
