heat and swelling usually indicate infection.. Have they done xrays since the surgery? perhaps a bit of metal fromt the surgical tools broke off in the wound and was not seen.Or the infevtion could be in the bone. My Mother in law had oral surgery done and ahd trouble with pain after wards ,they kept telling her it would get beter, it didn't, they finally did an xray and found a pocket of infection in her jaw bone the size of her thumb ,it had been there all along while they poo poo'd her.It required surgery to clean out the infection pocket. Also, has he been tested for Lupis? Yes it is more previlent in females, but men can get it if their female relatives have it in the line. Just throwing out ideas,good luck, melody
