After you make ACD changes you must activate those changes...
For NAT traversal you can get a Siparator from Ingate (~$1,000 for a Siparator 19), you could try OpenSBC (http://www.opensourcesip.org:8080/clearspacex/docs/DOC-1001), you could try to hack something together with OpenSer or you could wait for it to be built in to sipX (coming with version 4).
