Tried a low level format on a PC with a good SCSI controller? Adaptec 2940 PCI
single bus narrow controllers with BIOS are dirt cheap.
Don't need anything but the controller, any PC board with PCI slots, a
keyboard, power supply, monitor and any sort of video card. Great thing for
this minimal rig is a PC board with built in video.
I've salvaged many SCSI drives that were impossible to get working with any
formatting software on a Mac, because it's not possible to do a true low level
format with the built in SCSI on any Mac. The System has to be running, which
doesn't allow the direct hardware access to the drive needed to low level
format.
Dunno if any 3rd party SCSI card for Mac can do it.
