I always have blind people who I help with i devices set it to voiceover first thing. Once that's done, if speech is lost for any reason such as accidentally turning it off, you can trippleclick home and get Voiceover back in normal operation. If you don't do this and speech is lost for whatever reason, you may need to use iTUNES to sort things out and restore speech without sighted assistance.
