Yes, as Ronni's mentioned it sounds like you'd you set it up then migrated 
after that, as that's normally what happens with the "name conflict". 

It's pretty easy to reset it up (well fairly easy). Just run a Time Machine 
backup and perhaps a second bootable clone backup as well (to cover all the 
bases). Once happy everything is backed up restart the machine with Option key 
held down and boot into recovery partition. 
Erase with Disk Utility then choose "reinstall OSX". It will take a while as a) 
it has to download the whole 10.8.4 4GB image again off the Internet then 
install it. So that can take 2-8 hours or more depending on your Internet and 
congestion of servers (download servers). 
It will then install and restart and you have the choice to restore (you can 
then choose to restore from a) your clone or b) Time Machine backup. 
It will then restore all your data etc. 
After that, all should be good.
