PHP and MySQL Web Development, 2nd edition Luke Welling, Laura Thomson
I'm managing to work through it happily enough and would recommend it based on progress so far, i.e. about page 200.

This is a good second step - I use it too - but it still assumes a little knowledge. For absolute beginners I stand by what I said weeks ago when Joseph was starting out, that the Dummies php/mySQL guide's the best starting point for someone who has absolutely zero knowledge at all.
