It was most likely an unforeseen accident. Until the investigation commission brings in its report we really don't know what happened. Nope Columbia was the only shuttle that could not dock to the ISS. The mission was not equipped with the docking bay. Also there was no robot arm on this mission, so they could not inspect the underside. Moreover the mission was not equipped for conducting EVA's. If you look at the recent budget proposals advanced by the Bush administration, there were even deeper cuts to NASA's funding. Hey lets give away billions but very little to NASA.
