Are you saying this was ALL operator error?  (Don't anyone be 
embarrassed, I have done MY share
of operator errors, and blamed it on hardware, software or whatever 
until I figured out I did it all
by myself!)  And, it certainly is easy enough to foul things up when 
using multiple coordinate systems.

If it is not all operator error, but requires some manipulation of the 
coord. systems, maybe that will
help unravel what, exactly, is going on.
