I don't know if this is the same problem we had, but sounds very similar to a problem we had. There are three files in /var/lib/nfs that we had to remove: etab, rtab and xtab. If you remove them, they are recreated on boot. I have about 60 machines around the building that were on the fritz, off and on, I went crazy for a week. That was the fix, but I can't tell you how we discovered that those files were the problem or why they were "corrupted". Unfortunately we didn't document it. :^(
