[ozmidwifery] questions about care options in sydney

Hi,

I am fairly new to this list, I hope that this is the right place to be asking these 
questions. I had my first child 18 months ago in Adelaide, at the Birth Centre at the 
QEH. We received truly excellent care throughout my pregnancy, the birth and with 
breastfeeding problems afterwards, I wouldn't hesitate to go back but we are now 
living in Sydney. I am not yet pregnant but I am feeling anxious to know what my 
options are and a girlfriend who lives round the corner has just found out she is 
pregnant so we have decided to explore all the options available to us together. I 
would like to know:

1) is there a publicly funded caseload/continuity of care program running anywhere in 
sydney?

2) is there any publicly funded homebirth otpion available in sydney? 

3) where do I find a list of independent midwives working in the eastern suburbs?

4) does the RWH birth centre allow you to book as a private patient under the care of 
an independant midwife? If so is there a limited list of midwives who have admitting 
rights? The same questions for the KGV birth centre actually.

5) where can I find out stats on the birth centres at Randwick and KGV such as 
transfer out rates (before and during labour), intervention rates for transfered women 
and also any policies I am likely to want to know about (for example do they have 
policies regarding active management of 3rd state, water birth, breech, twins, vbac, 
time limits, etc).

6) who is the right person to contact in order to make a time to see the RWH and KGV 
birth centres and talk in person about their philosophies and policies?

7) I would also like to make a shortlist of good OBs should one become necessary  so 
anyone on this list who is able to recommend an OB practicing preferably at RWH/POWP, 
but anywhere in sydney would do, who is very supportive of natural birth and is for 
example homebirth friendly and experienced with vaginal breech, please let me know. I 
would also be very grateful for warnings of anyone to particularly steer clear of.

Hi Enthusiasts of MBC,

It is not an easy task to confront the medical 
establishment with pure 
facts that are contrary to what they hold as being true. 
Australian maternity care is not being practiced according to 
world best practice standards (WHO). Our rates of intervention are way too 
high and our outcomes are not 
satisfactory by any means. However, more to the point, 
to lose MBC would be to erode choice 
for women in childbirth - a choice for a better outcome 
and a better start to life.

We are at the point where we need to have a RALLY to 
show the Minister for Health that the community abhors her decision to limit 
choice.

We would love as many people - kids, mums, dads, 
preggie bellies, friends, midwives, clients - to get together outside 
the MBC and show their support for MBC, one-on-one midwifery, community 
midwifery, woman-centred care and choices in childbirth. Balloons, placards 
and enthusiasm!

We will be posting a date soon. It will need to be 
an early weekday morning at about 10AM. Probably a Wednesday.

Thanks for that info Jo.  Your comment that many of these 'doing it
unassisted' quite successfully is what I expected.  It concerns me that Jan
is seeking out information re any adverse or 'near miss' outcomes from any
PLANNED UNATTENDED HOMEBIRTHS to use as a  list of 'horror stories' that
ASIM will use to try and fast-track the funding of homebirths in NSW.

Just like how they try to use horror stories of midwife assisted homebirth
to tell us that we should be birthing in hospitals.

In fact, I think that Jan might find the opposite is true of what she is
seeking.

 As the coordinator of Homebirth Access Sydney (consumer run organisation)
I
 do receive calls from women requesting information about unassisted
 homebirth. I was receiving quite a few calls for a time there (averaging
1 -
 2 a month, which I consider alot), however I haven't received any for a
 while.

 It seems there are a range of reasons women are choosing unattended
 homebirth, it is important to remember here that unattended does not
 necessarily mean unsupported. Many of the women I have spoken with who
have
 opted to go down this road are very supported (just not by a midwife) and
 have studied and researched extensively and know more about birth than
most
 birthing woman and possibly some midwives.

 The reasons I have come across are (in order of preference)
 - Have had previous homebirths and don't have access to a midwife (either
no
 one in area or midwives they have contacted are totally booked up)
 - A personal philosophy, something that they and their partner really want
 to do
 - Have had previous negative experience in hospital, don't want to go down
 that road again but can't find a midwife.
 - Have had previous uncomplicated natural birth in hospital, want to birth
 at home but can't afford/find a midwife.

 It is very difficult because of the nature of phone contact there is often
 no follow up, therefore we never end up knowing what these women decide to
 do and what outcomes have occurred. I do ask them to keep in touch but
 inevitably they don't.

 Looking at the statistics of homebirths with a midwife attending and
 homebirths without a midwife attending (which I believe is more than
 double), it appears that many of them are 'doing it unassisted' quite
 successfully.

Dear Jo,
I'm glad to hear that you will still be involved in cares, although it
saddens that you will be calling it a day. However, time out to re-energise,
look after yourself and your family is important. Sometimes you have to put
other things first. Sometimes, new blood with fresh energy is needed while
others recharge their batteries. I'm absolutely certain that you would have
made a difference for many women's lives. Making a difference is something
to be very proud of and something that you can carry with you always. We
will miss you here on the list. I'll keep happily referring tassie women to
cares and birthrites as usual. The women love the fact that these are Oz
sites and not american! I really look forward to hearing from you with
feedback about the VBAC package that I put together for tassie women wanting
a VBAC.

You are posing excellent questions prior to making your decision.
I would recommend posing these questions in person with the relevant
managers of each of the birth centres in the south eastern Sydney area
health service.
King George is now known as Royal Prince Alfred Hospital and the direct
line to the birth centre there is 9515 5284  located at Missenden Rd,
Camperdown.

Royal Hospital for Women is 9382 6116 located at Barker Street Randwick.

St George Hospital Birth centre is 9350 3103 located at Belgrave Street
Kogarah.

I'd suggest visiting and talking with the midwives at all 3 birth
centres for a truly informed decision making process.

For Independent midwives working in the Eastern suburbs I'd recommend
telephoning Jan Robinson on 9546 4350 and Betty Vella on 9540 4992 and
Robyn Dempsey on 9888 7829 and Akal Khalsa 9660 2127

I'm sure there are lay/traditional midwives out there assisting women.
We're always really careful to promote attended homebirth and only refer
women to midwives who are registered, however I'm sure it wouldn't be hard
to find some traditional midwives, if that's what a woman is seeking.

My 
third son was fully breastfed at 13+ months, he just refused to eat. I think his 
first food was sucking a lamb chop bone, now at 21 months he eats a range of 
foods including breastmilk. The best thing was I didn't have to prepare baby 
food, and he did his jaw excercises (development stuff)munching on a toy 
or something. I was able to go by my own instincts on what he ate,with the first 
two boysI listened to others and their expert opinions, but like minded 
friends are a good source of information. I did know someone who introduced 
solids working ona philosophy about the first food being from highest off 
the ground and ending up with that which grows closest, such as grains etc, but 
don't know much more than that.
If she has gone this 
long without too much worry then she is probably doing really 
well

We 
watched the show, not too bad for ACA, but I don't think a few mins really does 
it any justice. Most think your mad, and its not about suffication of baby, more 
about the old "rod for yourback" syndrome. I got the best wakeup hug this 
morning form my bed buddy, a "rod" I have gotten very used 
to.

Hi Megan - a lovely wee "rod" by the sounds 
-Interesting hpw they cut and dramatise when you know what was said and how they 
have split sentences etc - I actually was "chopped" mid sentence when I said 
"there are no risks" I actually added "whenthere is a safe sleeping 
environment". I have a facts sheet on the ACA website - safe co-sleeping 
(seeing they were banging on about risks Ithought Id try for a second bite of 
the cherry!) -this facts sheet links to my site - and a list of co-sleeping 
links!!!

The lovely Chinese mum was my daughter's 
unihousemate.

i beg to differ - actually some babies wont touch solids despite them being 
available -my daughter (now 23 and a healthy vegetarian who has no eating 
disorders or food fusses and didnt as a child)had two siblings to 
copy and was always at the table on my lap at mealsand offered food -this 
baby wouldreach for a toy and chew on itbut refused any offer of 
solids until 11 months.She had been a colicky baby who reacted to 
foods - particularly citrus in my diet up to 6 months so I surmised she was 
possibly allergic -yet at the later age she didnt react or refuse any foods once 
she started to gradually eat.

Ihave known lots of babies within La Leche League (I was a group 
leader for 14 years) who took solidsmuch later (in the second half of 
their first year) and only ever saw one who showed signs of lethargy -this bub 
had a vegan mum and after the mum had B12 injections the baby remarkably became 
energised and crawled etc within days. All the other bubs who started foods 
later were healthy happy kids and didnt have any food fights or fussiness as 
toddlers-I think the parents attitude (ie forcing)creates more 
eating problems since it can be more of a power reaction/ battle. When we are 
relaxed and following the baby's lead - whether it be with eating or sleeping 
etc - we avoid battles over such natural functions.

I do agree though that good old fruit and veges would be a great 
start.

What ever happened to good old vegetables and fruit?
Mashed Vegi's and stewed fruit would be a good start - I used to 
pressure cook the vegi's - use the juice to mash them and then cool what 
was left of the juice as a very healthy drink - my babies loved 
it.
Also mention if left too long to introduce new tastes and 
experiences,itmay cause eating problems, digestive 
problemsand eating disorders.
My 4 yr old boy started on mashed vegi's at about 6 - 7 months now 
loves broccoli and peas and my daughter started eating a little younger 
and at nine she eats anything - asparagus, brussel sprouts, pumpkin, 
salad, letuce, celery, tomatoes - not so keen on lollies.
She was prem and started solids at a corrected 3 months, actual 6 
months and has absolutely no dietary or eating problems. My son 
was full term and is so fussy compared to her but still pinches the 
broccoli off everyone elses plate, have to eat quick or he gets 
it. LOL.

From about 6 to 8 months they do need to introduce some 
foodeven if it is just one spoon a day - it is important to start 
with something.
