The syntax is very different for CentOS 3.x vs. 4.x.
For both:
Enter "qeth" for the network type (not "eth")
For 3.3, the syntax is:
add_parms,0x10,{lo_devno,hi_devno},portname:port_name
i.e:
add_parms,0x10,0x0e38,0x0e3a,portname:CHIP00
For 4.4, when prompted for the subchannels, it's:
0.0.
