I had this issue with 4.1.1, upgrading to 4.2.0 fixed it.
You might be able to create the usage databases manually by looking at the

It's probably best to try and wget the URL from the Secondary Storage VM,
this will be where it is failing.
Also check that the Secoundary Storage is mounted on that VM.

Looking at the logs it almost seems as like the VPC is stopping
unexpectedly.
Looking at my VPC, the first interface is a Link Local for Control type
traffic, which is running on KVM.

It looks like your issue is with the local storage, as opposed to resources.
Personally, I don't know exactly what 'can not find local sr' means but I'm
sure someone can chip in.
In the meantime, I would check what you have defined as primary storage on
cloudstack and that it is functioning

/var/log/cloud was the old logging location before v4 (I think!), logs are
now at /var/log/cloudstack/management/
management-server.log is the most useful I find, if there isn't anything
immediate there attach it and I will take a look for you.

directories, for example /var/log/cloudstack-management is made, but I have
never seen anything in it.
