I've always had problems hearing the voicemail. I solved the problem was having Siri play the voicemail.

When you delete voicemail messages, they go into a holding area called Deleted
Voicemail. You should find a button for it on the voicemail screen. Maybe you
haven't deleted messages from there. If that's the case, open Deleted
Voicemail. It includes a Clear All button you can use to
