I think they can be different without a wall or lintel. Example 4 heads in a
square room. Heads 8' off the wall on 3 sides and 9' off on the 4th side.
Heads 16' apart. The two heads nearest the 9' off the wall are 18' x 18' and
the other two heads are 16' x 16'. I don't think this is a usual scenario but
certainly legal. Can even be more complicated if the room were 28' wide and
46' long. Heads 7' off the wall on 3 sides and 9' off on the 4th side. 16'
between the first two head in the 46' direction and 14' for the next two heads.
Now you have 18', 16', and 14' in the same room. You probably only do this due
to aesthetics and something preventing you from spacing between heads at 16'.
Simple s x l application is all that required.

I don't think so if your design basis is 18x18 and the small (I'm seeing a dog
leg off the living room for the kitchen) compartment directly communicates with
the larger no matter how small the smaller compartment is. 18x18 rules
throughout. Now if it's a separate compartment, with at least the lintel, then
that rule wouldn't necessarily apply.
