And there are 3 places I can think of to look:

1.) Admin costs (automation, standardization, et al)

2.) Patient-Provider. Do you really need to go to the doctor today?
If you go for a cough do you need an MRI and CAT scan?  Yes, no, who
decides?  This is where consumer directed health care can save
zillions: both in educating consumers to make smart choices and in
encouraging doctors to align their fees to market demand.

3.) Preventative care.  Stop smoking, stop eating crap, start
excercising, et al.  And if you choose not too, then you pay for the
privilege.  Should be just like gasoline: if you want 450hp V10 engine
then you pay more to travel.

My fear would be that we go in the direction of standardizing on plans
rather than payer.

I think this is how Canada and the UK do it and that will create the
long lines, the crappy care, etc.
