Lack of sharing information I think is the big reason. Yes, CIA knew about this. But it never got to the NCTC.
I have a relative that his a Federal LEO. He says that even today, the sharing of information among the Federal level is still lacking. Also, there is technology out there that would have helped catch this guy at the airport.
