When you're busy working on something like a modification, it is just simply not possible to always know everything at all times. You can catch bugs at any point in development. Death messages have just recently begun to be a problem because our beta testers (around 60 or so people) have been complaining about the lack there of. I never noticed the death messages not being there, because, well, I work with developer enabled *always* - I just didn't happen to figure out that the death messages did not show up when developer wasn't enabled.
Who would? Not with a mentallity of it working all these months.
