I don't know if this is directly related to your problem.  I have been using 
Firefox 1.5.x on OS X
and Windows for quite some time.  I have installed a few extensions, 
particularly the Netcraft
Anti-Phishing Toolbar.  When visiting the http://toolbar.netcraft.com site 
there is a link button
for installing the toolbar.  This opens up an alert bar at the top of the page 
to indicate that
the install option was blocked.  There was a link which could be used to open 
up a dialog.  A list
of domains which were trusted is displayed.  A button allowed the 
toolbar.netcraft.com domain to
be added to this list.  Afterward, repeating the process to download and 
install the toolbar
worked.  I was advised to restart Firefox (I assume this means closing all 
instances).  The
toolbar was available on the next launch of Firefox.

It sounds from this thread that this procedure is not working for you.  I'm 
guessing that you
would have figured all of this out on your own.  I wonder if some directory 
used by Firefox has
the wrong ownership or permissions and it can't be written to by your web user. 
 I would not be
surprised if these were in the home directory since each user would have their 
own desired
toolbars and other extensions.

One problem I have had with Firefox on the abovementioned systems is related to 
viewing images in
Yahoo email.  Normally the "view" and "download" links have different actions.  
One will preview
in the browser and the other will download.  In Firefox the "view" insists on 
downloading.  It
offers a dialog to select an external application (even though it could display 
a jpeg internally)
for viewing or save to disk.  It says you can change preferences to make the 
selected choice work
all the time.  I have never been able to get this to "stick" so I always have 
to go through a two
or three-step process.  It also often forgets the selected application.  It's 
one of my few
annoyances with Firefox.

I would like to thank you for mentioning pgrep.  It is an interesting function. 
 I have long been
in the habit of using `ps aux | grep "something"` to locate running processes 
by name or keyword.
