"GSM 02.07 - Mobile Stations (MS) features":
  B.1.11 International access function

Provision is made for a direct, standard method of gaining
international access. For this purpose the MS may have a key whose
primary or secondary function is marked "+". This is signalled over
the air interface and would have the effect of generating the
international access code in the network. It may be used directly
when setting up a call, or entered into the memory for abbreviated
dialling. This feature is of benefit since the international access
code varies between CEPT countries, which might cause confusion to a
user, and prevent the effective use of abbreviated dialling when
roaming internationally. Users may still place international calls
conventionally, using the appropriate international access code.

"GSM 02.30 - Man-Machine Interface (MMI) of the Mobile Station (MS)":
The physical means of entering the characters 0-9, +, * and #
(i.e. the SELECT function) may be keypad, voice input device, DTE
or other, but there must be means to enter this information.

This is mandatory ("may have a key" only means that there may be other
ways to enter this information, the keypad is optional). If this didn't
work then you wouldn't have a GSM phone :-)

BTW, somebody have obviously forgotten to remove the CEPT reference.
GSM is also used in quite a few non-CEPT countries nowadays, giving
even more variation...

And the numbering plan is of course operator independent in most
countries, so the international prefix is a national matter and not
something an operator can choose. See
<URL:http://www.pts.se/nummerplan/nrplan.htm> for the Swedish plan.
