This problem is related to differences between theorical model and real 
world : theorical model is just a restricted set of things from real world.

Are gravity variations created by underground density variations handled by 
the d20 rules ? No, because it would lead to unplayable rules, although 
these variations exist in reality, and, if I remember, can be used to 
detect underground water.

This is a question of balance between realism and playing fun. The 
d20-system is not the most realistic, but I think it's acceptable. If you 
are looking for more realism, look at ICE's system.
