I had a similar problem. I wanted a plated through hole on a 2 layer board but only placed a pad on the top layer. There was a ground plane on the bottom layer, and the hole shorted to it. DRC did not detect the problem, and the board houlse did not catch it. On the next design I placed a bottom layer pad having the same diameter as the hole, and the polygon plane poured around it correctly.
