Surname. From the surname list you can edit each surname and make the change

Similarly Great Britain, although more difficult to define, hasn't
ever even included Ireland as I understand it.

The primary reason for this is because you would lose the flexibility if you
want to switch between ALL CAPS or mixed case. If you type in mixed case,
you have the option to display in ALL CAPS. If you type in ALL CAPS, you
don't have the option to display in mixed case.
