for listening to things, the higher end Etymotics headphones are good at
blocking outside noise. At a much lower pricepoint Decibulz has molded earphones
that block as well or better than the etymotics and are pretty good sound.
Decibulz is just comeing out with a bluetooth version (the kickstarter ones
haven't shipped yet https://www.decibullz.com/product/decibullz-wireless/ )
The problem with all of these is that the mic is a 'standard' earphone mic on
the cord, so it's not good at other people hearing you.
Years ago I tried the Jawbone bone-conduction earpieces. They worked well when
they were in place, but I had a horrible time getting it to hold in place (it
needed a lot of pressure against the side of my head to work)
Recently I've been using the headbones bone conduction headphones

They do not block sound (I wear standard earplugs for that). I find that in
really noisy environments I have to press the left side against my head, and
mute the phone when not talking.
A couple years ago I tried a throat mic like

but found that with my phone, the audio from the mic was too loud and distorted
badly and my nexus 5 phone didn't have any way to adjust the mic gain so I
couldn't make it work well. It was great at eliminating outside noise into the
mic. It's earpiece was only 'ok', but you could easily wear earphones over it to
block the signal.

Fortunately for my wallet, I never needed them that badly (except twice
on IBM support calls) and now I'm not on the hardware side any longer.
