I just got a pair of Turtle Beach. They have active noise cancellation
that appears to work really well. I haven't used them in the D.C. yet,
but they work really well. They do not have a boom mic, but they do have
some kind of reasonable built-in mic. They come multi-device Bluetooth
capability as well as:
remote answer
remote hangup
remove mute
turn on/off active noise cancellation
remote volume control
also works with standard physical audio cable (comes with it)
comes with USB based charger
very comfortable
remote base and treble boost
Not cheap.
It remains to be seen whether it works well in the noisy DC on the 'can
you hear me' front, but it claims to have active mic noise cancellation
as well. I haven't tested that function, but the rest works pretty well.
I was on a call today in an office where other people were talking and
the rep that I was talking too didn't appear to notice.

The problem with all of these is that the mic is a 'standard' earphone
mic on the cord, so it's not good at other people hearing you.
Years ago I tried the Jawbone bone-conduction earpieces. They worked
well when they were in place, but I had a horrible time getting it to
hold in place (it needed a lot of pressure against the side of my head
to work)

They do not block sound (I wear standard earplugs for that). I find
that in really noisy environments I have to press the left side
against my head, and mute the phone when not talking.
A couple years ago I tried a throat mic like

but found that with my phone, the audio from the mic was too loud and
distorted badly and my nexus 5 phone didn't have any way to adjust the
mic gain so I couldn't make it work well. It was great at eliminating
outside noise into the mic. It's earpiece was only 'ok', but you could
easily wear earphones over it to block the signal.

Fortunately for my wallet, I never needed them that badly (except twice
on IBM support calls) and now I'm not on the hardware side any longer.
