I have checked the detailed SMTP logs and this message was only deliver to the mail server once.

Look through the full delivery headers for the message, including Message-ID and Received headers, and make sure that it's not a problem with the same message actually being delivered to the mailserver multiple times. That's actually more common in my experience.
