Someone (finally) noticed that apreq's test suite isn't compatible with
Apache 2.4 and requested a change. Given that we haven't released an
updated apreq in nearly 6 years, I'm inclined to make/test the changes
to the test suite and immediately go to a release cycle.

I haven't tested this with the latest Perl/mod_perl/httpd stack
(although this worked fine as-is last time we did a release).
While I dig into this by me (may take some cycles), would you explain

libapreq passed the 2 sets of C-based tests and failed the 3rd set
(quite miserably), so it may just be a bug in Apache-Test. I'll look
into it and send a proper bug report with details to apreq-dev
I tend to use 2.0 where I need subversion or PHP (which ship with 2.0
modules only) and 2.2 everywhere else... I personally find the naming a
bit tricky (2-2.2). and would frankly rather see the modules called
mod_perl20 (or mod_perl2.0) and mod_perl/libapreq22/2.2; I'd rather type

Well, the spool file is just a normal apr_file_t, and the actual file
handle is accessable via apreq_brigade_spoolfile(), so you'll likely
want to do something like:

apr_status_t rv;
rv = apr_file_name_get(&filename, apreq_brigade_spoolfile(param->upload));

after using SVN mod_perl.

Having utterly no idea what it was you said on IRC yesterday, I can only
comment that it looks much less cluttered, at the cost of losing most
upward links to the parent httpd project (and it takes a few moments to
