I think many swap out the original CD drive for an Apple one to avoid driver issues. You really do need 3rd party software for the drive to function fully otherwise. I think the reason that it sorta works if there's a disk in the drive at boot is that there may be some limited driver somewhere (possibly in the device) that loads under those conditions, kinda like being able to run a Zip drive without loading a driver at boot with a disk inserted, or running a monitor on a PC at 640x480 with limited colors without the proper software. It won't do audio CDs for the same reason. There used to be a hack of one of Apple's drivers available. If you can get a hold of a copy of that, you might be in business as well.
