 Dictionary. The purpose of this form is to provide a mechanism for PCORnet
 data partners to communicate information about how they transformed their

 Strategies may vary from site to site. For example:
   - UNMC can deploy changes to their EMR quickly enough to embed mappings
 there, so that their i2b2 concept codes are based on standard terms (e.g.
 LOINC:xyz); so all they have to do is put those concept codes in the
 `c_basecode` column of the CDM-in-i2b2 metadata rows.
   - KUMC doesn't expect to deploy mappings in their EMR until later in the
 project, so their i2b2 concept codes are those codes native to the ERM
 (e.g. COMPONENT_ID:nnn). These component IDs can be assigned to children
 of the CDM-in-i2b2 concept paths.

 In either case, a query that uses the CDM-in-i2b2 terminology will find
 the relevant patients and data.

 I believe I've mapped i2b2 - CDM at least enough to answer the Data
 Summary questions in the PCORnet ETL Annotated Data Dictionary spreadsheet
 (see ticket:144#comment:14).  For performance reasons, I ended up using
 tables for the CDM objects rather than views.  See also

 I realized last night that the unit tests for the Data Builder (#134 #87)
 include code to generate a mock i2b2 star schema. It's currently very
 limited, but I had an idea:

 Make a spreadsheet with diagnoses, the labs etc. used to diagnose them,
 and the meds etc. used to treat them (insulin).

 ||= Condition =||= Indication =||= Treatment  =||
 || Diabetes||A1C   ||   Insulin||
 || Diabetes||BMI   ||  ||
 || MI  ||   Pulse  ||   Aspirin||
 || Sepsis  ||  ||   Antibiotic ||

 And use those as the basis to generate test encounters.

 Refinements:
   - value distributions for labs etc. (mean, std dev)
   - probabilities to go with the relationships
   - sequences of related encounters: normal results, abnormal, diagnosis,
 treatment, normal

 data structures necessary to do so.

 Drs Jeffrey Statland, John Chen, Scott Koertner, and Wendy He had a
 meeting this afternoon, discussing the progress of the ALS cohort
 characterization and future steps. Jeff gave a lot insignts on what would
 be interesting information for this patient population. He will come up
 with a list of prioritized variables for data query. Besides demographics,
 stand of care will be the focus of the list.

 Some trivial notes
 Demographic variables (of particular interest):
 occupation (employed or not)
 Livng situation (along or live with others, marital status) (found
 this variable in Heron but not many patients have the information)
 Economic status (may come from insurance information)

 Standar of care
 Medication (Only one FDA approved medication for ALS, but there
 are many medicaitons for specific symptoms, such as drooling, muscle
 spasm.
 Device: bipap
 Found ALSFRS in flowsheet and Pba (pseudo bulbar) score. Jeff will
 find out when the clinic started using these scales.

 Others
 Jeff thougt that the 1900+ ALS cases in Heron is more than
 expected. (more than half of them are dead though. But the 800 alive ones
 are also more than expected based on the patients seen in the clinic)
 John mentioned that a patient need to have at least 2 ALS
 outpatient encounters be be qualified as a ALS patients (this may reduce
 the patient volumn)
 Jeff mentioned that it is important to find out what happended to
 patients before they were diagnozed with ALS (This is regarded as research
 question that we can work on later)

Although there is no specific diagnosis date for ALS in Heron. It
 would be interesting to show the distribition of the time difference
 between first ALS encounter and time of death. The expected median is 3
 years.

 If the clinician orders the medication in Epic/O2, it should be in HERON.
 I'm not sure if this is the same as the prescription workflow or not.
 If you query based on medication(s) in HERON (without specifying a
 modifier) you should be including patients who ever had an order recorded
 in the Epic system for the selected medication(s) whether or not a
 dispense was recorded.

 Is there a way finding out whether last.visit is the last fulfilled visit
 or the last scheduled visit?  I think it is important for research
 purpose.

 If it is the last real visits with potential entry errors, then it is a
 quality issue.
 If it is the scheduled last visit that may not be fulfilled, then it is a
 validity issue as we may no use it to calculate survival time depending on
 % of last scheduled visits unfulfilled.

 Shortly after I left for vacation, I received response from Regenstrief
 with the additional LOINC codes that were needed for standardization of
 CDM V1.  I have attached a spreadsheet with LOINC coding for CDM V1.
 Hubert and Mike will deploy the rest of the metadata at Nebraska and issue
 a shared copy on Babel for all GPC members to employ.

 Yes , only difference is I'm running it on my Mac as the native OS and
 using VMWare Fusion . I didn't have to install a second Network adapter ,
 the first was able to get to the internet and download the required
 packages.

 Sure I'm using the same heron_extract.zip that you used ? I assume so . I
 may try a fresh install again , this time with the pg_hba.conf mod first
 before the installation .

 Note that our research Clarity instance at UNMC is in SQL Server and our
 i2b2 databases are in Oracle.  This particular view lives in SQL Server
 because of how we have to divide our work up - our Oracle Dynamic Gateway
 for SQL Server has habit of toppling over on queries that are complicated
 so some of our processing is done in SQL Server.  This is one example of
 that.

 Our source data at UNMC has considerable more detail regarding
 administrative types of encounters and you should tell us if you have
 needs for tracking those types of events.
 The Completed encounter types are actual face-to-face encounters of some
 type with the patient and are those you presumably what you want for
 research.  The others are administrative or scheduling encounters and
 should be ignored for patient care issues

 Thanks for sharing the details of UNMC's approach to encounter types
 (attachment:pcori_encounters.sql:ticket:178 and ticket:178#comment:6).
 It's reasonably clear how that query lets you compute PCORNet CDM summary
 stats on encounter type.

 But we still have to decide what encounter terms GPC will use.

 The UNMC metadata on babel (as of Nov 6 ticket:64#comment:26) doesn't seem
 to include the results under UNMC: Encounters. Do you plan to continue
 working in that direction?

  Thanks for the clarification. There is a variable called last.visit
 among the demographic variables in Heron. What we wanted to find out is
 whether last.visit is a fulfilled visit or not. If yes, then we know the
 patient was last known alive on that day. if no, then we can't use
 last.visit as last known alive date.

 It is great to know that encounter type can differentiate encounters
 completered and others. But this is not enough for answering the original
 question. When we run a query, we pull encounters related to a specific
 disease only, so the last encounter of the patient with the hospital
 (likely related other diseases) may not be in the pulled data.

 I don't know whether the date of last.visit corresponds to  the last
 encounter (among all encounters with the hospitals). If it is, then we
 need to find that encounter and its type (completed or not). Dan and
 Tamara: do you know whether we can do this?

 If last.visit is not linked to an encounter in the system, then we are
 back to the begining ot the original question: is last.visit fulfilled or
 scheduled only.

 needs for tracking those types of events.
  The Completed encounter types are actual face-to-face encounters of some
 type with the patient and are those you presumably what you want for
 research.  The others are administrative or scheduling encounters and
 should be ignored for patient care issues

 The screenshare demo went well today. I talked through the
 BuilderSaga#QuickStart screencast, pausing to address questions and
 issues.

   - George @ MCW was able to follow along and get it working; whatever
 problems he saw earlier didn't show up today.
   - Angela @ UTHSCSA had various issues that should be addressed by
 starting from a clean i2b2 VM
   - Phillip and company @ UTSW emerged pretty confident they could get it
 working, though they weren't quite in sync today.
   - Keith @ WISC joined a little late and didn't follow step-by-step but
 seemed confident he could manage.

 After the basic data builder install, we continued beyond the GPC
 requirements into the [comment:16 SEINE] technique for delivering data via
 REDCap. The new portable sqlite3 architecture doesn't provide this
 functionality yet; we plan to finish it in the upcoming monthly HERON
 release. So Dan and Nathan demonstrated the current R-based production
 workflow at KUMC.

 George has the ball to provide whatever revisions to BuilderSaga seem
 worthwhile for documentation.

  I think Secondary is a bit of a misnomer; I think it just means not
 primary, and there can be may of them. The database column is just a Y/N
 for primary.

  It seems reasonable to me that if your system doesn't distinguish
 primary from secondary, you should just report the more general term, e.g.
 Billing Diagnosis. Or if you don't even know that much, just don't add a
 modifier at all. For studies that require the distinction, MCRF would have
 to evaluate the benefit of participating against the cost of adjusting
 your ETL.
 
  But I think I should escalate this issue, as it bears on
 DataRepositoryManagement, which is included in the GPC data sharing
 agreement by reference.


 Am I missing where we would differentiate a source of insurance? Structure
 seems like it would match Encounter Diagnoses. Could be good to define
 now, if the goal is to incorporate health plans in the future.

 That's correct - in the Cerner Millennium Problem List the valid statuses
 are Active, Cancelled, and Resolved, and Cancelled would be used for the
 use case you are describing re: Deleted from Epic.

 We keep the Problems List and Diagnoses List separate (you can turn
 problems into diagnoses in the chart, of course, but on the database side
 there are distinct and separate tables (PROBLEM and DIAGNOSIS). For
 modifiers on diagnosis codes, we only specify between primary and non-
 primary, as Laurel described earlier in this thread, so I wouldn't vote
 for Secondary as the modifier of choice. We are using Primary and non-
 Primary in our current i2b2 projects. It's not reflected in Babel yet
 because we haven't finished the deployment but I'll have an updated
 diagnosis ontology in December.

 Never mind then. Call me when you can. So when i2b2 1.6 came a long with
 modifiers, we enhanced HERON to use modifiers to make the distinction in
 our Sep 2012 release:

  The CDM spec says Please do not include diagnoses generated from
 problem lists, which are a different workflow and data-generating

  We've also heard this same request on the Cerner side, but the
 meaningful research distinction has always been with Primary vs. Non-
 Primary and no further granularity has been brought to my attention thus
 far.
  Regarding problem list diagnoses, do you mean problems that have
 been deemed diagnosis-worthy and therefore are only those diagnoses that
 originated from the problem list? If that's the case, I know that the
 workflow exists in Cerner to capture that, but I will have to do some more
 digging to find out how we capture that in our extracts and how it might
 be brought in to i2b2. Currently we do not have that flag in i2b2, if it
 is for diagnoses that originated on the problem list. The alternative
 would be records on the problem list that are coded with diagnosis codes,
 which on the clinical side don't make them diagnoses, but could cause
 confusion on this topic.

 Due to travel and other meetings, John, Jeff, Scott, and Wendy finally met
 all together on 21st (Scott and Wendy met on Nov 7, Scott, Wendy, and John
 met on Nov 14).
 The major issue for ALS cohort characterization is to identify variables
 needed. We went through a list Jeff made in October but many of them do
 not identify enough records. At the meeting, we decoded to use ICD9 code
 to identify variables for comobidities. John provided a list of ICD9 codes
 for creating charlson comobidity code and Jeff will find ICD 9 codes for
 other variables. Identifing medications remains an issue.

 Plans for the breast cancer survey (KeyGoalTracking#bc-survey) are in
 flux, and in some versions, deploying the data builder at each site is

 Comments from Scott Campbell at UNMC

 I have created a hierarchical view of the Lab LOINC items for
 approximately 2000 of the 2500 terms used at UNMC.  This also includes
 orderable codes (i.e. batteries) and performable codes (tests/results).
 The hierarchy is based on LOINC class and refined by UNMC departments and
 logical groupings.  Note:  we have generalized the UNMC departments into
 categories that should make sense to most medical centers.

 Jim Campbell is reviewing my categories to ensure that they are in-line
 with a clinician’s perspective.  Based on Jim’s feedback, I will refine
 and issue a testable code set to Hubert.
