If you firewall your outgoing connections, make sure there is no traffic
from the server being blocked. Had this problem once and in the end I
found out I missed one outgoing port.

Do you renice the processes? There is a known issue with nice and the
source engine.
Other then that the only thing i can think of is the custom maps that
you are running. Maybe try on a standard map for debugging.

Seems the server is unable to get their steam ID. Checking the firewall
log shows that no packages are being dropped at all.
Does anyone have any ideas on what might be causing this? I only have
the problem on this one server, so I'm sure it is something I did as
opposed to a bug.

Think he means a failover kind of solution, where if one connection
fails, you can still use the other.
