I used the javascript/dhtml one by SNITZ at one time. It seemed pretty
thorough and easily integrated into a site. However, that was almost two
years ago so I don't know if it's still around, or what it's like if it is.

Have you tried running a virus protector, to clean the system? This
appears to be a variant of the w32.hybris virus.

Usually those Gotcha programs disable themselves if you put the mouse in
the top left or right corner. I doubt it's a virus, just a stupid
program made in VB or something.
You'll have to look in msconfig (regedit if your on 2K or NT) and remove
the program if it runs on startup.
Without the knowledge of the exact problem, it could be a virus. But if
it is a program then running the task manager will help identify this. I
had to recently clean a machine that the desktop icons would run away
any time you moved the mouse near them, and is the reason I made my
suggestion.
There's a function in VB (and other languages) to remove it's entry from
the task manager, so that wouldn't necessarily work.
Another possibility is that it attached itself to a system program that
runs on startup, which is what the system file checker is good for if
your on win98. 2K and XP
There's an open source Fusebox one at
http://fbopenforums.sourceforge.net. It's got support for Access, SQL
Server and Oracle at the moment.
