I can see it happening if the secondary servers cannot allocate memory for
the re-cache. Is your primary server in development cache mode and the
secondary servers aren't?
You would think if one malloc'd then they all would -- unless the memory
or ARS configuration differs...

I contacted BMC regarding this issue over a year ago and they identified
it as a defect around the GUID on the form -- there is a unique index on
that field. Their workaround was to remove the index. They never
identified the cause of the issue.

There is a defect logged on this. It doesn't work from a filter. Contact
support -- I'm pretty sure it broke from 6.3 - 7.x and am unsure if it is
resolved in 7.1

Not without having a user trigger the add via an active link, which does
work.
The ardbc filesystem plugin could work, but I haven't gone there yet.

We made the first tier of OpCat required.
The downside is you have to have an Other -- or make sure you've captured
every possible categorization. No data is better than bad data.

The server should recognize any valid MAC addresses in the keys. Use the
one from the physical card (even if it's not the one prepopulated in the
license dialog.)

Except that it's just an animated GIF that spins. It has no correlation
to work actually being performed, or a percentage of completion being
reached.

BMC Remedy Administrator displays junk characters when showing application
license usage.
I only post because there was just a thread around this exact issue.

Win2k3R2 here.
Were you using the native dll that you can download during the install?
I've found that it trashes tomcat on some of my machines but not others.
I've renamed it on all of them. tcnative.dll
