That's exactly what I do and teach. I insert the shank into router collet
until it "bottoms out" (won't go in any further). I then lift it back up in
the collet about 1/8 to 1/4 inch. Have never had one get stuck and never had
one come loose either! There is/are one or two router bit

Apply a "dot" (small dab?) of "Blue Loctite" threadlocker to the set screw in
the extension.
That should resolve your issue. If you only use that one bit in the extension,
you may want to try the Red Loctite (semi-permanent locking) on it.

and tailstock center. If it were, he'd have eliminated all the flats at very
close to the same time - versus still having a flat
